Three tails and gills on the abdomen indicate that this is a mayfly nymph although in this specimen the gills are all missing except on the very first abdominal segment. The broad head and flattened body place this nymph in the family Heptageniidae. The hind margins of the first thoracic segment are drawn out into rear pointing projections and this places it in the genus Ecdyonurus. Further characteristics would need to be examined to determine the species.
