Hairy woodland caddis.

Looking again, the forewings on this small caddisfly are closer to 9mm than 11 as stated below. Either way they still fall into the size range for C. villosa, 6-12mm. The other species mentioned is 8-10mm but it has a more pointed rear end to the forewing and is not just so broad as this going by the info. and pictures in Barnard and Ross.
