insect larvae or caterpillar

It is very small, probably about 1cm or a bit larger when it stretches out. I thought it was a caterpillar, but not sure as it is a similar shape to a leech, as it tapers in towards the head. It moves by sticking its mouth to the ground and pulling up the rest of its body towards it. It often has its back end stuck to the surface and moves and stretches out its head as if searching for something.
