I have been told that the white spots on the top are the remains of the sheath that housed it before it reached its current maturiy. Is this right?

The sheath you refer to is the so-called universal veil, which is ruptured by growth. You can often see A. muscaria just past the point of rupture, still veiled, but with bits of red peeping through.
