16cm tall. gills off-white. bell shaped and scaly cap approx 6cm wide. veil.
found in old hay meadow. single specimen.

have you got a pic showing base of stem too. its probably a lepiota but just wanted to rule out amanita.
no pic but no volva either - if thats what youre looking for.
my initial impression was Lepiota too. on closer inspection felt closer to Agaricus hence my allegiance shift!
I think the death cap is ususlly found near conifers.
