You would only confuse it with the harlequin if you ignored the fact that the harlequin is twice as big!
As a general rule with ladybirds it is a good idea to have something else in the photo to give scale now there are so many harlequins around, photographing it on your hand was very useful in helping to identify this as a two-spot ladybird.
